Calcified amorphous tumor of the heart.

چکیده
A 44-year–old previously healthy man presented with progressive shortness of breath on exertion. Chest x-ray showed clear lung fields, but dense calcification was noted within the cardiac silhouette. Sixty-four–detector row cardiac computed tomographic angiography revealed diffuse calcific infiltration of the left ventricular (LV) myocardium, which also involved the papillary muscles and mitral chordal apparatus (Figure 1 and online-only Data Supplement Movie I). Calcific infiltration was most notable in the apical, lateral, and septal free walls. The right ventricle was spared except for contiguous involvement of the calcific process at the mid portion of the interventricular septum. The LV systolic function was severely depressed, with a volumetric calculated ejection fraction of 20%. Transesophageal echocardiography (online-only Data Supplement Movie II) similarly demonstrated diffuse calcification affecting the mitral chordal apparatus and LV myocardium with noninvolvement of the mitral annulus and mitral leaflets. On laboratory analysis, serum parathyroid hormone, calcium, creatinine, and glucose levels were within normal limits. No hypereosinophilia was noted by complete blood count. Endomyocardial biopsy of the LV was performed for histopathological evaluation of the calcific process. The extensive calcification of the LV myocardium was well visualized in the catheterization laboratory (Figure 2). Yellow-white sections of tissue were obtained by biopsy, and histological analysis showed fragments of dense collagenous tissue covered by thickened endocardium and myocyte hypertrophy but no evidence of malignancy. Congo red stains for amyloid were also negative. On the basis of the pathological findings and the irregularly shaped calcific cardiac mass demonstrated by computed tomographic angiography, the patient was diagnosed with calcified amorphous tumor (CAT) of the heart (Figure 3). Surgical excision was avoided because of the extent of calcific infiltration, and he was referred to be evaluated for possible heart transplantation. Cardiac CAT is a rare nonneoplastic cardiac mass that mimics malignancy and causes symptoms due to obstruction or embolization of calcific fragments.1,2 In the present case, the diffuse calcific infiltration was associated with congestive heart failure symptoms. Surgical excision is the treatment of choice in suitable candidates and can be curative. Postoperative recurrence of CAT has been reported, and patients should be followed vigilantly.3 Disclosures None.

Calcified amorphous tumor of the heart (cardiac CAT) is a rare non- neoplastic cardiac mass with unknown pathogenesis, 12 cases of which have been reported so far. This report presents a 73 year old woman with chest pain complaint continuing since last year. Workup revealed left atrium mass of posterior wall origin . The specimen was a solid calcified nodule with firm consistency and 2.5cm in d...
متن کاملCalcified amorphous tumor of the heart with mitral annular calcification: a case report
BACKGROUND Calcified amorphous tumor of the heart is a rare, non-neoplastic cardiac mass characterized by nodular calcium in the background of amorphous degenerating fibrinous material. Clinical diagnosis of calcified amorphous tumor can be difficult, and current single imaging techniques do not specifically differentiate calcified amorphous tumor from other cardiac tumors such as calcified atr...

INTRODUCTION Cardiac calcified amorphous tumor is a rare, non-neoplastic intra-cavity cardiac mass composed of calcium deposits in a background of amorphous degenerating fibrinous material. Only a few cases of this rare lesion have been reported in the available literature. Clinico-pathological differentiation of this lesion from calcified atrial myxoma, calcified thrombi or other cardiac neopl...

Calcified amorphous tumor of the heart consists of a cardiac mass of rare nonneoplastic nature that mimics malignancy and causes symptoms due to obstruction or embolization of calcific fragments. We present a case of tumor 17-year-old young, male, in tricuspid valve, with classic pathological findings. It was preferred to approach for classic median esternotomy, installation of the circuit of e...
متن کاملSurgical removal of calcified amorphous tumor localized to mitral valve leaflet without mitral annular calcification
A cardiac calcified amorphous tumor (CAT) localized to the mitral valve leaflet without mitral annular calcification (MAC) is a rare entity. We report a case of a 69-year-old woman with such a condition, who underwent successful excision of the tumor and mitral valvuloplasty using a glutaraldehyde-treated autologous pericardium. During 38 months of follow-up, no recurrence of a cardiac mass has...
متن کاملA Rare Case of Cardiac Calcified Amorphous Tumor: Multi-Modality Imaging Evaluation
BACKGROUND Cardiac calcified amorphous tumors (CAT) are rarely presented and featured as calcification and eosinophilic amorphous material in dense collagenous fibrous tissue. CASE REPORT Our case report describes a 47-year-old man presenting cardiac CAT with only chronic cough and occasional dizziness. Preoperative multi-modality imaging was used to evaluate it and postoperative histological s...
